At Her Strength Training Studio Inc., the top priority is to provide a safe, effective, and personalized training experience. To ensure this, all new clients must complete an intake process before beginning. This is standard practice and helps us design the best program for you.

 

Step 1:

Client Intake Forms

Paperwork isn’t the fun part, but once we get through this  step, we can focus entirely on your progress and success!

 

Confidentiality Assurance:

Her Strength Training Studio Inc. takes your privacy seriously. All information provided on the intake forms will be treated with the utmost confidentiality and will only be used for the purpose of creating a safe and personalized training plan. Your personal details will not be shared with any third parties without your consent. Rest assured that your privacy is a top priority, and Her Strength Training Studio Inc. is committed to maintaining the highest standards of confidentiality.

Required Intake Forms and Descriptions:

PAR-Q+ (Physical Activity Readiness Questionnaire)

 A screening tool used to determine if medical clearance is required before starting an exercise program. If clearance is needed, consult with your physician or healthcare provider to assess your readiness for physical activity and to receive any necessary guidelines.


Health & History Questionnaire  

This helps assess past injuries, and any health considerations, allowing a safe and effective program to be designed tailored to your needs.


Informed Consent & Waiver

Before we get started, it’s important to understand that exercise, while beneficial, does come with some inherent risks. This form is here to help you make an informed decision about participating in our sessions. While every effort will be made to minimize these risks, it's important to know that by participating, you’re acknowledging the potential for injury and agreeing to take responsibility for your health and well-being.

I’m here to guide you safely and help you reach your fitness goals, but ultimately, your participation is voluntary and at your discretion. 


* Please note, for the consultation to be scheduled, all forms must be signed, completed, and returned, along with any required doctor’s clearance. We appreciate your understanding and cooperation.​​

Step 2:

Consultation & Program Design

Once we receive your completed forms and any necessary physician clearance, we’ll schedule a free virtual consultation to discuss your goals and fitness history. If you choose to move forward with personal training, you’ll sign the Client / Trainer Agreement, which covers scheduling, cancellations, and payments. Your personalized program will then be created based on the information you've provided.
